A company is looking for a Behavioral Health Care Manager to join their Collaborative Care team.
Key Responsibilities Coordinate patient care and serve as the primary contact for patients in the Collaborative Care program Conduct patient assessments, provide education on mental health disorders, and deliver brief behavioral interventions Document patient progress, track clinical outcomes, and adjust treatment plans as necessary
Required Qualifications Bachelor's degree in social work or a related behavioral health field Specialized training in brief psychosocial interventions and motivational interviewing Minimum of 2 years of experience in behavioral health or care management Preferred clinical license (e.g., Licensed Mental Health Counselor, Licensed Social Worker)
Experience with screening for mental health and substance use disorders
