Program Coord - Iop​/Php

Responsibilities

• The Clinical Care Coordinator, under the direction of the program manager, is an active member of the patient treatment team.
• Provide milieu management and continuous patient care supervision, interactions and role modeling to patients.
• Provide support at the front desk, including:
  • Maintaining patient transportation schedule, including scheduling Lyfts as needed
  • Answering phones
  • Completing phone assessments with potential clients and adding them to Windmoor Call Log
  • Providing required paperwork for intakes, discharges, and weekly outcome measures
  • Entering outcome measures in Reflections tracking database
  • Supervising and updating daily attendance tracker
  • Tracking patient attendance, absences, and length of treatment
  • Following no-call/no-show protocol as needed to ensure patient safety
• Meeting with potential stepdown patients from inpatient units for registration.
• Providing support to clinical staff by completing admission assessments, treatment plans, and/or facilitating group sessions.
• Assisting patients with obtaining resources and referrals as needed.
• Participating in additional tasks as needed through assignment by the program manager.

• Graduate degree in social work, mental health counseling, marriage and family therapy, or related field.
• A minimum of 1 year experience working with individuals with persistent mental illness (PMI) and/or substance abuse.
• CPR certification and successful completion of restraining procedures training within 30 days of employment.
