Home Health Nurse

Duration: 3 months with possible contract extensions

Seeking an experienced and dedicated Home Health Clinician to provide high-quality care to patients across designated coverage areas. This role involves delivering patient-centered care in a home health setting, ensuring compliance with clinical standards, and maintaining strong documentation practices.

• Shift: Monday – Friday
• Hours: 8:00 AM – 4:30 PM
• Weekly Guarantee: 40 hours

• Mileage reimbursement provided between patient visits (from first to last patient)

• Deliver skilled home health services in accordance with physician orders and care plans
• Develop, implement, and revise individualized care plans
• Provide patient and caregiver education to support disease management and recovery
• Maintain accurate, timely, and compliant documentation in Homecare Homebase (HCHB)
• Collaborate with interdisciplinary teams to ensure coordinated patient care
• Monitor patient progress and report changes in condition to appropriate parties
• Ensure adherence to regulatory and agency standards

• Prior Home Health experience (mandatory)
• Proficiency in Homecare Homebase (HCHB)
• Strong experience with OASIS documentation
• Ability to manage a caseload independently and efficiently
• Excellent clinical, communication, and organizational skills

• Strong time management and scheduling flexibility
• Ability to work autonomously in a fast-paced environment
• Patient-focused approach with strong interpersonal skills
• Familiarity with local coverage areas (preferred)
• Adaptability and problem-solving mindset

• No specific certification required (state licensure applicable to role is expected)
