As a Field Registered Nurse, you will:

• Complete comprehensive patient assessments, including physical, functional, psychosocial, and environmental evaluations.
• Develop, implement, and update individualized plans of care to promote positive patient outcomes.
• Determine patient eligibility for home health services and complete admission and recertification assessments.
• Educate patients and caregivers on disease management, medications, nutrition, safety, and self-care.
• Coordinate care with physicians and interdisciplinary team members.
• Supervise and provide clinical direction to Home Health Aides and LPN/LVNs.
• Perform skilled nursing interventions, treatments, and patient education.
• Monitor patient progress and communicate changes in condition promptly.
• Maintain accurate, timely documentation in accordance with regulatory and agency standards.
• Ensure compliance with infection prevention practices and payer requirements.
• Advocate for patients while helping them maintain independence and improve quality of life.

Required

• Active Registered Nurse (RN) license
• Current CPR Certification
• Valid driver's license
• Reliable transportation with current auto insurance

Preferred

• Minimum of 2 years of Home Health nursing experience
