Per Diem. Full‑time or PRN positions are available.

• Provide nursing care in accordance with the patient’s plan of care, including comprehensive health and psychosocial evaluation, monitoring condition, health promotion and prevention, coordination of services, teaching, training, and direct nursing care.
• Coordinate total patient care by conducting comprehensive health and psychosocial evaluation, monitoring patient condition, promoting preventive practices, coordinating services, and teaching and training activities.
• Evaluate nursing service effectiveness to patients and families on an ongoing basis.
• Perform admission, transfer, re-certification, resumption of care, and discharge OASIS for the home care patient.
• Prepare and present patient records to the Clinical Record Review Committee as indicated.
• Consult with the attending physician concerning alterations of patient care plans, obtain approval from supervisor, and make changes as appropriate.
• Coordinate patient services and submit daily tally of patient care visits.
• Participate in case conferences, discuss problems with supervisor, and determine best handling strategies.
• Consult with supervisor regarding need for additional health team members (home health aide, PT, ST, OT, social worker, etc.).
• Cooperate with other agencies to provide continuity of care and implement comprehensive care plan.
• Participate in staff development meetings and pursue continuing education through workshops, conferences, professional organizations, and research.
• Assist in development and periodic revision of physician's plan of treatment and process change orders.
• Submit clinical notes within 48 hours, progress notes, and other clinical record forms outlining services rendered.
• Participate in patient discharge planning process.
• Maintain knowledge of current drug therapy and adhere to federal, state, and accreditation requirements, including Medicare and Medicaid regulations.
• May be requested, by the Clinical Manager, to fill in for other nurses.
• Conduct initial and ongoing comprehensive assessment of patient’s needs, including OASIS assessments at appropriate time points.
• Obtain medical history from patient and/or family.
• Conduct physical examination including vital signs, physical assessment, mental status, nutrition, etc.
• Evaluate patient, family, and home environment to determine health teaching and availability of assistance.
• Determine need for Home Health Aide services and frequency.
• Explain nursing and agency services to patients and families as part of care planning.
• Develop and implement nursing care plan and initiate preventative and rehabilitative procedures as appropriate.
• Observe signs and symptoms, report to physician changes or reactions to treatments.
• Teach, supervise, and counsel patients and caregivers on nursing care needs at home.
• Assume responsibility for care provided by Home Health Aide; supervise and evaluate at least every 14 days.
• Submit written evaluations to appropriate department or individual for Home Health Aides in region.
• Participate in conferences with Home Health Aide supervisor regarding aide performance.
• Chart services rendered, changes noted, revise nursing care plan as needed, evaluate patient progress, and close charts of discharged patients.
• Discuss with supervisor need for other health team members.
• Obtain orders for home health aide service and submit referrals.
• Provide guidance, supervise, and evaluate LPNs once monthly.
• Cooperate with other agencies to provide continuity of care and implement comprehensive care plan.
• Participate in educational experiences for student nurses.
• Participate in planning, operation, and evaluation of nursing service.
• Prepare care plan for Home Health Aide.

• Graduate from an accredited School of Nursing.
• Licensed in the state of Florida as a Registered Nurse.
• One or more years of experience in community/home health agency or hospital setting is preferred.
• Knowledge of Medicare and Medicaid guidelines.
