WHAT YOU WILL DO:
The Elder Services Case Manager is a non‑exempt position responsible for caseload management within the elder population, which may include Adult Protective Services High Risk referral. This position coordinates services, develops care plans, and implements and monitors service delivery.
- Provide case management services in accordance with guidelines and requirements as established by the funding source
- Screen, assess, and evaluate clients for service needs and develop and implement care plans to address service needs
- Evaluate client eligibility for programs, including assistance with Medicaid eligibility
- Maintain regular contact with clients as established in program guidelines
- Provide information, referral, and follow‑up for appropriate support services, including coordination of client services with Medicare or Hospice agencies
- Resolve service delivery problems
- Complete and submit accurate and timely documentation, including but not limited to maintaining client records
- Report any suspected abuse, neglect, or exploitation to the Florida Abuse Hotline, according to the State of Florida statute
- Maintain established productivity requirements as established by the supervisor and as related to caseload assignment
- Perform all other duties as assigned
- $22.33 per hour
- Paid Time Off (PTO) is based on the number of hours worked.

- Bachelor’s degree in social work, sociology, psychology, or a related social services field
- Bachelor’s degree in a field other than the social services field with professional human services experience may substitute on a year‑for‑year basis for the education requirement if approved by the funder
- Must have two (2) years of case management experience preferred
- Considerable knowledge of community reorganizations and resources
- Effective verbal and written communication skills
- Must be efficient in using Microsoft Office applications, Word, Excel, Outlook, Teams
- Ability to lift and /or carry case files, office supplies, donations, etc. up to 25lbs
- Potential exposure to a stressful environment involving difficult client situations, including clients who may be verbally and/or physically aggressive
- Must have a valid driver's license with auto insurance coverage.
