Overview

This position will lead the Mount Vernon School-Based Health Center, located at Mount Vernon High School. The provider in this role delivers comprehensive primary care services to children and young adults ages 0–22 within the Mount Vernon City School District, as well as students and families in surrounding communities. The role is year‑round with potential for modified hours during school breaks and summertime based on operational and patient care needs.

The provider functions with a high degree of autonomy and is supported by a small team. A minimum of 2 years' experience in primary care, urgent care, or a related clinical setting is required.

Essential Functions

Assess patients and/or populations; develop, implement, and evaluate plans of care; clearly document and communicate information both in written and verbal forms. Perform diagnostic and therapeutic procedures according to established guidelines and standards as necessary for the care of the patient. Utilize evidence‑based practice, serve as a clinical expert for policies, procedures, and documentation, and assist in the education, training, and development of new and existing staff members.

Engage in the “community of the work unit” by staying aware of changes, participating in data collection or auditing, and engaging in quality improvement. Leverage technology and informatics to optimize health‑care outcomes. Commit to continued professional development of self and others. Contribute to division and specialty goals and the hospital strategic plan. Education & Licensure

Master of Science degree or equivalent required. Licensed as an Advanced Practice Registered Nurse (APRN) or Physician Assistant (PA) in the state of Ohio.

Certifications

Maintain national certification as an APRN in the appropriate population focus for the position, or PA.
