Job SummaryWe are seeking a skilled Home Health Physical Therapist (PT) – PRN – Per Visit – Home Health to join our team in Dallas, TX. The ideal candidate will have experience in providing physical therapy services to patients in their homes. As a Home Health Physical Therapist, you will be responsible for assessing patients’ needs, developing treatment plans, and providing therapy services to help patients regain their mobility and independence.
Responsibilities- Conduct initial assessments of patients’ physical abilities and develop treatment plans.
- Provide therapy services to patients in their homes.
- Monitor patients’ progress and adjust treatment plans as needed.
- Communicate with patients’ physicians and other healthcare professionals to coordinate care.
- Document patients’ progress and maintain accurate records.
- Provide education to patients and their families on how to manage their conditions and prevent further injury.
- Current license to practice as a Physical Therapist in the state of Texas.
- Minimum of 1 year of experience in home health.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Strong problem‑solving and critical‑thinking skills.
- Valid driver’s license and reliable transportation.
