As a home health physical therapist, you visit patients across the Cleburne area and deliver hands-on care. Typically, your day includes:

• Evaluating patients and building personalized therapy plans
• Guiding patients through strengthening and mobility exercises
• Tracking progress and adjusting care as patients improve
• Teaching patients and families safe movement techniques
• Documenting each visit accurately and on time
