Responsibilities
  • Ensure that all activities performed align with the vision of Elara Caring’s board of directors, executive team, and the leadership of the Hospice team.
  • Assess hospice care patients and families to identify the physical, psychosocial, and environmental needs of patients as evidenced by documentation, clinical records, interdisciplinary team reports, after‑hours reports, and on‑site evaluations.
  • Assess patient needs and obtain data on physical, psychological, social, and spiritual factors that may influence patient/family/caregiver health status.
  • Communicate significant findings, problems, and changes in health condition, environment, or unsafe facility conditions to the Clinical Supervisor, physician, facility, and/or other personnel involved with patient care.
  • Counsel, instruct, and include the patient, facility, and family in following the interdisciplinary plan of care.
  • Teach hospice philosophy and pain and symptom management to staff in alternate care settings (nursing homes, hospitals, assisted‑living facilities, etc.).
  • Provide appropriate support at time of death and perform bereavement assessment.
Required Qualifications
  • Graduate of an accredited certificate, diploma, associate, or bachelor’s school of nursing.
  • Current state license as a registered nurse (RN).
  • One year of experience in a clinical care setting.
  • Experience in a hospice or home health environment is preferred.
  • Fifty‑percent travel required.
  • Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
  • Valid driver’s license and reliable transportation to perform job tasks.

You will report to the Clinical Team Manager.
