Job SummaryThe RN will support primarily chronic care and wellness patients in the child‑bearing years from pre‑pregnancy to postpartum through the mobile (four+ counties) and fixed (Abbeville) BLOOM clinics. He/She will provide in‑person, telehealth and remote patient monitoring (RPM) care, lead RPM operations for the BLOOM Clinics, support clinical research, and perform other related duties.
Key Responsibilities- 25% Nursing support:
Provide nursing assistance for the CRH nurse practitioner, including patient education, workup, blood draws, surveys, history collection, and documentation of clinical encounter notes. - 25% Remote Patient Monitoring enrollment:
Serve as first contact for RPM enrollees, conduct outreach, enroll patients in person or virtually, document medical and social determinant information in the electronic health record, obtain consent, provide devices, and communicate program details. - 25% Remote Patient Monitoring management:
Create care plans, monitor eligibility, review enrollment data, communicate readings outside normal parameters to patients and providers, manage standing orders, follow up on non‑compliant patients, and troubleshoot issues. - 15% Medical supply inventory management:
Maintain inventory of RPM devices, order supplies, pre‑register devices for mobile events, and assist with insurance requirements for replenishment. - 10% Sponsored programs support:
Assist clinic staff and principal investigators with approval of research protocols, clinical research implementation, and completion of training on protection of human subjects.
- Graduated from an accredited school of nursing.
- Clinical rotation experience in patient care coordination and/or RPM.
- Current unrestricted RN licensure and CPR certification.
- Minimum of 1 year nursing work experience.
- Master’s degree in Nursing.
- 2–3 years clinical experience with emphasis on care coordination, RPM, and telehealth; rural health clinic experience a plus.
- Stand for prolonged periods (50%).
- Walk or move about (50%).
- Use hands or feet to operate or handle equipment (30%).
- Ascend or descend stairs and ladders (5%).
- Position self to accomplish tasks such as stooping, kneeling, crawling (10%).

Band: C09 (expected range: $60,700.00 – $77,000.00)
