Job Summary
The RN will support primarily chronic care and wellness patients of child‑bearing years from contemplating pregnancy, pregnant, and postpartum through the mobile (about four + counties) and fixed (Abbeville) BLOOM clinics. Supports in‑person care, telehealth care and remote patient monitoring. Leads RPM operations for the BLOOM Clinics and supports clinical research. Performs other duties as assigned.
Job Duties- 25% – Nursing Support Provides nursing support for the CRH nurse practitioner and BLOOM operations, including patient education, workup, blood draws, surveys, patient history, and documentation for clinical encounter notes. Also provides education and counseling of patients up to limits of RN license.
- 25% – Remote Patient Monitoring Enrollment Serves as first point of contact between Clemson Rural Health and RPM enrollees, performing outreach, enrollment, consent, device education, and documentation.
- 25% – Remote Patient Monitoring Management Establishes care plans, monitors eligibility, reviews enrollee data, communicates with patients and providers, manages status changes, and performs persistent follow‑up to meet program requirements.
- 15% – Medical Supply Inventory Management Maintains RPM device inventory, orders supplies, pre‑registers devices, assists with prior authorizations, and meets insurance requirements.
- 10% – Sponsored Programs Support Assists clinic staff and principal investigators in implementing research protocols, completing required training in protection of human subjects research.
- Education:
Graduation from an accredited school of nursing and related nursing experience. - Clinical rotation with patient care coordination and/or RPM.
- Current unrestricted RN licensure and CPR certification.

1+ years.

1st Master’s Degree in Nursing. - Work experience:
2–3 years in a clinical setting with emphasis on care coordination and/or remote patient monitoring and telehealth; rural health clinic experience a plus.
- Job Knowledge:
Firm working knowledge of concepts, practices and procedures, and ability to use them in varied situations. - Supervisory Responsibilities:

Standard
Hours:
37.5;
Monday‑Friday, 8am‑4:30pm.
Band: C09 (expected range: $60,700.00 – $77,000). Salary is dependent upon several factors including, but not limited to, a candidate's previous experience, knowledge, skills and performance in accordance with Clemson's compensation guidelines.
Essential Personnel LevelRequired to follow emergency facility closure and modified operations directives, and not normally expected to work on‑site during emergency situations.
