Care Manager, Telephonic Nurse

The Care Manager, Telephonic Nurse assesses and evaluates members' needs in a telephonic environment to achieve and maintain optimal wellness. This role is ideal for experienced nurses who are skilled in care coordination and resource facilitation.

• Pay: $71,100 - $97,800/year
• Location: Remote, SC
• Department/Unit: Care Management
• Schedule: Full-time
• Residency: Must reside in the state of South Carolina
• Travel: Up to 20% travel may be required

• Conduct telephonic assessments to identify members' clinical, behavioral, social, and environmental needs.
• Identify and resolve barriers that hinder effective care or treatment adherence.
• Develop, implement, and update individualized care plans as appropriate.
• Monitor member progress toward desired outcomes through ongoing assessment and evaluation.
• Provide education and support for chronic conditions management and self-care.
• Coordinate care and facilitate access to appropriate providers, services, and community resources.
• Collaborate with members, families, providers, and interdisciplinary teams to support care goals.
• Maintain timely and accurate documentation in accordance with established guidelines and quality and compliance requirements.
• Apply department, segment, and organizational policies, procedures, and operational objectives in day-to-day work.
• Exercise independent judgment and utilize available resources in managing assigned caseloads.

Education:

• Bachelor of Science in Nursing (BSN) preferred

Experience:

• 3+ years of recent acute care clinical nursing experience
• Experience providing education and support for chronic condition management
• Experience with community resource coordination

Licenses &

Certifications:

• Active, unrestricted Registered Nurse (RN) license in the state of South Carolina with multistate compact privileges and no disciplinary action required
