Location: South Clermont
Amedisys, Inc. is seeking an RN Field Case Manager for Hospice in Clermont, FL. This on-site role requires weekend travel within Lake County, with Saturday and Sunday, 8:00 AM - 8:00 PM shifts.
You will perform patient assessments, develop care plans with the care team, and coordinate referrals. Strong clinical RN experience, active Florida RN license, and CPR are required; hospice/home health experience preferred. Background screening required.
