RN Field Case Manager - Hospice (Clermont, FL)

This position is based in Clermont, FL and requires weekend travel within Lake County.

Schedule:

Saturday and Sunday, 8:00 AM - 8:00 PM.

Salary Range: $39-42 hourly, plus bonus opportunities. Final compensation may vary based on experience and skills.

Hospice RN Case Managers utilize and leverage:

• HCHB – Proprietary Real-time database
• Enclara Pharmacy solutions with 24/7 access
• IPrescribe – Efficiencies with medication management
• Forcura – Document management system
• Medline – Supply ordering system
• TalentEDGE – Amedisys  premier learning center
• EBSCO – Nursing Resource Center for CEUs
• Tablet with voice dictation and mileage tracking
• Outlook added to Teams, Chat, Yammer

You will then continue your on-boarding working directly with nurses in our program. On average our orientation for an RN is 7-9 weeks, however is individualized according to your needs and experience.

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other hospice care disciplines as needed or documents rationale for not doing so.
• Develops, prepares, and maintains individualized patient care progress records with accuracy and timeliness according to care center policies.
• Records pain/symptom management changes/outcomes as appropriate.
• Teaches patients, families, and caregivers appropriate patient care techniques.
• Regularly communicates with clinical manager and care team.
• Performs on-call responsibilities and on-call services to patients/families as assigned.
• Other duties as assigned.

Required:

• Current RN license in the state(s) where you will work
• Minimum 1 year of strong clinical RN experience in an acute healthcare setting (Required)
• Current CPR certification

Preferred:

• Hospice or home health experience
• Strong communication and interpersonal skills
• Ability to work independently and manage multiple priorities
• Comfortable with daily travel between facilities
• Proficient in electronic medical record (EMR) systems
