Lutheran Services Florida seeks a compassionate Family Case Manager to support children and families through coordinated social services. In this role, you will assess family needs, develop service plans, connect clients to community resources, and monitor progress to ensure safety and well-being. You will collaborate with schools, courts, and partner agencies, maintain accurate case records, and participate in team meetings. Responsibilities

  • Conduct comprehensive assessments of family needs, risks, and strengths.
  • Develop and monitor individualized service and safety plans for children and families.
  • Coordinate referrals and link families to community, behavioral health, and educational resources.
  • Maintain accurate, timely case notes, reports, and documentation in agency systems.
  • Collaborate with schools, courts, and partner agencies to support family stability and child safety.
  • Conduct home visits and ongoing follow-ups to track progress and adjust plans as needed.
  • Respond to crises and escalate safety concerns according to child welfare protocols.
  • Advocate for clients while upholding legal, ethical, and agency standards.
  • Participate in team meetings, case staffings, and training to support best practices.
  • Promote LSF's mission, values, and culturally responsive, trauma-informed care.
