SUMMARY
Incumbents provides assistance to seniors in securing resources to remain independent.
ESSENTIAL DUTIES & RESPONSIBILITIESThe list of duties and responsibilities is illustrative only and is not a comprehensive listing of all the duties and responsibilities performed.
- Maintains regular attendance and punctuality
- Performs other duties of a similar nature or level
- Assist and advise residents and families of the services which may be necessary to maintain a self-reliant lifestyle
- Promote wellness activities for senior residents
- Educate residents, families, and staff on available community resources
- Assist residents in building informal support networks among themselves and with family members
- Acts as a liaison between community agencies, service providers, and residents
- Encourage residents to be proactive in meeting their social, psychological, and physical needs
- Facilitate meeting of needs when necessary, but avoids the creation of unhealthy dependence
- Monitor the delivery of services to residents to ensure they are appropriate, timely, and satisfactory
- Report all suspected abuse situations to the appropriate agency
- Set up volunteer support programs with service organizations in the community
- Advocate and may negotiate on behalf of residents for adequate, timely and cost-effective provision of services
- Meet with service providers as needed and appropriate
- Maintain individual files on residents which will contain at least the following: intake information, service termination information, quarterly review and follow-up, human or civil rights abuse, resident/family meeting notes

- Establishing and maintaining effective working relationships with associate personnel, other City employees and the general public
- Ability to perform basic support skills
- Maintain a safe work environment
- Use interpersonal skills to interact with coworkers, supervisor, the general public, etc., to sufficiently exchange or convey information and to receive work direction
Bachelor’s Degree-Concentration in Gerontology, Psychology, Human Services, Public Health, or related field preferred; or an equivalent combination of education and experience sufficient to successfully perform the essential duties of the job.

PHYSICAL DEMANDSLight Work:
Exerting up to 20 pounds of force occasionally, and/or up to 10 pounds of force frequently, and/or a negligible amount of force constantly to move objects.
The work will occur in an office and field environment. While performing the duties and responsibilities the employee may be exposed to loud noise, noxious odors, unsanitary conditions, chemicals, electricity, moving mechanical parts, varying weather conditions, and other related conditions and situations.
