Position Overview

The QIDP is responsible for the overall program coordination within each home assigned. The QIDP will develop and implement Individual Plans (IPs) that promote self‑determination, development, and independence of those we serve. The role ensures that all systems and practices are followed routinely and meet the requirements of all regulatory agencies. The QIDP assists staff in performing all responsibilities related to the individual’s program services plan and is jointly responsible with the Residential Coordinator to ensure effective supervision of the group home.

This position is also responsible for ensuring compliance with federal, state, and local rules and the policies and procedures of New Avenues to Independence, Inc. The QIDP promptly reports to the supervisor any circumstance that negatively affects the health, safety, or well‑being of residents, the agency, or its equipment.

Responsibilities
  • Develop comprehensive I.P. through team process to meet the individual needs of those served and monitor active treatment programs and various professional interventions per I.P. goals and objectives. Coordinate, monitor, and ensure all aspects of an individual’s I.P. are implemented and properly documented.
  • Facilitate and participate in the process of individual admissions, discharges, and service substitutions with their residential sites.
  • Participate in the development and implementation of the behavior support programs in conjunction with the behavior support committee.
  • Advocate for individual rights and needs.
  • Possess current working knowledge of and comply with all regulatory agencies.
  • Communicate with all persons involved in the individual’s life.
  • Lead in MUI process.
  • Maintain and enter information and eligibility requirements into the DODD portal; e.g., LOC, DDP, etc.
  • Assist with QIDP responsibilities in the absence of QIDP’s as directed by Residential QIDP Manager.
  • Assume responsibilities in survey and inspection preparation with attention to compliance with expectations on a daily, ongoing basis. Assist with plan of correction for citations.
  • In coordination with the Core Team (Nurse and Residential Coordinator) supervise the Direct Support Professionals regarding I.P., skill building, and behavioral supports program services.
  • Participate in recruiting, orientation, and training of the Direct Support Professional (DSP).
  • Train staff regarding the professional interventions for skill building, active treatment, behavioral supports, and I.P. implementation.
  • Evaluate staff regarding professional interventions utilized in skill building, active treatment, behavioral supports, and I.P. implementation. Participate in the evaluation process with the Core Team.
  • Apply the Right Time Right Place concept with the Core Team in order to provide adequate presence, support, interaction and training of staff to address individual needs.
  • Participate in management responsibilities.
  • Assume on-call responsibility according to agency practices and expectations. Perform on-call duties with the understanding that these duties are a necessary and important support function.
  • Comply with all agency policies and procedures (i.e., Policies/Procedure Manual, Employee Handbook, etc.). Pursue continuous learning and professional development.
  • Perform all other duties as required.
Education and/or Experience

A Bachelor’s or graduate‑level degree from an accredited college or university; OR hold a professional license or certification issued by the Ohio board of psychology, the state medical board of Ohio, or the Ohio counselor, social worker, and marriage and family therapist; OR hold a certificate to practice as a certified Ohio behavior analyst pursuant to section 4783.04 of the Revised Code.

Licenses, Registrations

Must maintain current certification in Positive Behavior Supports (PBS), CPR/First Aid, Medication Administration. Must hold a valid Ohio driver’s license and possess a driving record that meets New Avenues to Independence, Inc. insurance carrier acceptability.
