We are currently seeking a APP (FNP, NP, Adult Gero NP or PA) based in Cleveland, OH,serving local skilled nursing facilities within a close-knit, mission-driven care community.

• Document efficiently using an EHR designed to streamline workflows
• Voice dictation [and AI Scribe] for fast and accurate documentation
• Access to Up To Date ® for instant clinical guidance

• 6-hour facility shifts for full time positions (rounds generally start between 7–10 a.m.)
• Freedom from on-call and overnight requirements
• Timely Charting using company-provided iPad with LTE

• Provide high-quality, patient-centered care to residents in skilled nursing facilities
• Conduct daily rounding, prescribe medications/treatment plans, and address acute/emergent issues
• Communicate with facility nursing staff via phone or secure messaging as needed
• Maintain reliable on-site attendance for scheduled shifts
• Ensure compliance with clinical standards, facility policies, and regulatory requirements

Advanced Practice Providers must hold an active U.S. license and be board certified by ANCC, AANPCB, PNCB, or AACN (for Nurse Practitioners), or by NCCPA (for Physician Assistants). Qualified specialties include but not limited to Internal Medicine, Family Medicine, Emergency Medicine, General Practice, Physical Medicine and Rehabilitation, and Geriatric Medicine.
