Director, Patient Care Services; Hospice

Our client, h/care Hospice, is looking for a hospice leader who can do both: deliver exceptional clinical care and grow a census in a competitive market. This isn't a role for someone who wants to sit behind a desk. You'll be the face of our Cleveland hospice, building relationships with physicians, hospitals, and families while leading an interdisciplinary team of nurses, social workers, and chaplains.

• Own the P&L for our hospice department: census growth, staffing, compliance, margin
• Build referral relationships with physicians, hospitals, and SNFs
• Lead and develop an interdisciplinary care team
• Ensure compassionate, high-quality end-of-life care for patients and families
• Compete effectively against larger hospital-system hospices

• 5+ years clinical nursing experience, 2+ years in hospice leadership
• Track record of growing census or building referral relationships
• Deep knowledge of Medicare hospice regulations and CMS compliance
• Comfort with EHR systems (Well Sky preferred)
• BSN required, MSN or Healthcare Administration master's preferred
