Care Management Supervisor

Location: Gordon Square Office, 5209 Detroit Avenue, Cleveland, Ohio 44102

Employment Type: Full-Time

About

The Role

We're seeking an experienced and compassionate Care Management Supervisor to lead our care management team in delivering exceptional whole-person care to our clients. In this role, you'll provide clinical and administrative leadership while ensuring our clients receive coordinated, comprehensive services that align with our mission and values.

As a Care Management Supervisor, you'll work collaboratively with our Site Clinical Director and interdisciplinary teams to screen, assess, and coordinate care for clients seeking services including primary care. You'll be instrumental in building team capacity, analyzing data to improve outcomes, and ensuring our services meet the highest professional and ethical standards.

What You'll Do Leadership & Team Development
  • Provide clinical and administrative leadership to the care management team including case managers, community health workers, and patient navigators
  • Orient, train, and mentor team members using the SLII model to develop self-reliant achievers
  • Conduct individual and group supervision to ensure professional growth and skill development
  • Recruit, interview, and hire qualified team members in partnership with Human Resources
Clinical Operations & Quality
  • Coordinate day-to-day clinical operations ensuring appropriate coverage and productivity standards
  • Monitor documentation, record-keeping, and service delivery through database analysis and quality reviews
  • Ensure adherence to ODMH administrative rules, federal/state laws, and professional standards
  • Conduct clinical assessments and provide direct services to clients as needed
Care Coordination & Collaboration
  • Partner with the Senior Director of Population Health to analyze data and coordinate outreach services
  • Manage transition of care notifications and assign appropriate staff follow-up
  • Collaborate with internal and external providers to ensure seamless, integrated care
  • Co-facilitate team meetings and client care conferences
Program Management
  • Monitor and report on program outcomes in collaboration with site leadership
  • Ensure timely service delivery and proper utilization of resources
  • Maintain transparent communication through meetings, email, and interpersonal engagement
  • Participate in committees and assume supervisory functions as needed
What You'll Bring

Required Qualifications
  • Bachelor's or Master's degree in Social Work
  • Minimum 3 years of related work experience in a non-profit, health-related organization
  • LSW (Licensed Social Worker) or license eligible within 6 months of accepting the position
  • Strong clinical knowledge of mental illness, substance use disorders, and therapeutic modalities

Physical Demands

This position operates in a professional office environment with regular sitting, standing, walking, and computer use. Travel and work outside normal hours may be required for meetings, trainings, and events.
