About the RoleThe PSH Behavioral Health Therapist provides integrated clinical services to adults living with serious mental illness, with a specialized focus on schizophrenia-spectrum disorders and individuals residing in permanent supportive housing. This role combines direct therapeutic care, case coordination, crisis support, and community-based outreach — all grounded in a trauma-informed, recovery-oriented framework that honors each client's autonomy and strengths.
You will work as part of a dynamic multidisciplinary team — collaborating with housing staff, case managers, medical providers, and community partners — to ensure holistic, person-centered care across clinical and housing environments.
What You Will DoDirect Clinical Care
Provide individual and group therapy using evidence-based practices tailored to adults with serious mental illnesses, including schizophrenia-spectrum disorders. Conduct comprehensive mental health assessments and develop individualized treatment plans in collaboration with clients and the care team, centering client voice and preference throughout. Deliver services with a recovery-oriented, trauma-informed, and person-centered approach that supports autonomy, dignity, and community integration.
Care Coordination and Housing Support
Coordinate care across behavioral health, medical, and housing systems, acting as a liaison among providers, case managers, housing staff, and families. Address behavioral health needs that impact tenancy, including crisis intervention, symptom management, and medication adherence. Collaborate actively with housing staff and property management to support clients in maintaining housing stability and accessing wraparound services.
Crisis Response and Risk Assessment
Provide crisis intervention and risk assessment, including suicide and violence risk evaluation. Coordinate access to higher levels of care when clinically indicated, responding with urgency and compassion in emergency situations.
Documentation and Compliance
Maintain accurate and timely documentation in compliance with agency, funding, and licensing standards. Participate in regular interdisciplinary team meetings to ensure continuity of care and shared decision-making.
Professional Growth
Engage in ongoing training and professional development to maintain licensure and stay current on best practices in behavioral health and supportive housing.
What We Are Looking ForLicenses and Credentials
An active LSW or LPC license in the State of Ohio is required. An LISW or LPCC is strongly preferred. A Master's degree in Social Work or a related social service discipline is required, along with a minimum of three years of experience in mental health and chemical…
