About This Role
The RN Patient Educator – Diabetes Care and Education Specialist at Cleveland Clinic's Main Campus is responsible for coordinating the diabetes education program and providing educational resources to patients and caregivers. This role is ideal for a dedicated professional looking to make a significant impact in diabetes care and education.
Highlights- Pay: $69,600 - $106,133/year
- Location: Cleveland, OH
- Facility: Main Campus Hospital
- Schedule: Full-time, Day shift
- Assist with the overall coordination of the diabetes education program.
- Provide educational resources to physicians, caregivers, and patients.
- Facilitate the organizational change process to promote and implement services.
- Provide direct patient care using medical nutrition therapy or diabetes self-management education.
- Collaborate with Endocrinology providers to design and administer nutrition and diabetes care.
- Complete comprehensive assessments for each patient, including emotional and behavioral health.
- Develop individualized care plans with safe and effective interventions.
- Utilize national standards and guidelines for diabetes and obesity care in practice.
- Deliver specialized interventions and comprehensive counseling and education services.
- Implement care plans, communicate plans, and maintain documentation.
Education:
- Bachelor's degree or higher in nursing, dietetics, or a related field
Experience:
- One year of experience as a Registered Dietitian or Registered Nurse
Licenses &
Certifications:
- Current license to practice dietetics or nursing in the State of Ohio
- Certified Diabetes Care and Education Specialist (CDCES) upon hire or within two years of employment
- Dietitians must have completed a dietetic internship and have active certification as a Registered Dietitian (RN) or Nutritionist (RDN)
