A Brief OverviewThe Clinical Pharmacy Specialist for SCC Tripoint will practice at the newly renovated, state-of-the-art facility located east of downtown Cleveland, serving residents of Lake, Geauga, and Ashtabula counites. This new facility will serve as an extension of the Seidman Cancer Center located at the academic main campus. In this role, you will join a highly collaborative outpatient care team dedicated to delivering comprehensive and patient‑centered pharmaceutical care across multiple oncology disease states.
In this role, you will provide direct ambulatory oncology clinical services, including therapy management, patient education, toxicity assessment and monitoring, and optimization of supportive care.
You will work closely with physicians, advanced practice providers, and nursing colleagues to support individualized treatment plans, advance evidence‑based practice, and contribute to quality‑improvement and practice‑development initiatives. This role also includes opportunities to precept learners, including APPE students and PGY‑1 and PGY‑2 pharmacy residents, helping to support the growth and development of future oncology pharmacists.

- Provide Clinical Pharmacy Services (drug information, drug therapy monitoring, patient specific consultations, healthcare staff education, pharmacoepidemiology and pharmacoeconomic evaluations in selected patient populations)
- Participate in Medication Safety and Therapeutics (MS&T) Committee process: evaluate requests for new drug usage and old formulary drug relevance and review therapeutic classes for relevance, assist in maintaining and updating the system formulary
- Serve as an informational and patient problem solving resource for Clinical Pharmacists and other healthcare professionals
- Provide continuing education to Clinical Pharmacist staff
- Documentation of clinical activities
- Serve as a preceptor for pharmacy residents and pharmacy students
- Actively participate in local and system committees and work groups pertaining to the Clinical Pharmacy Specialist’s area of expertise
- Performs other duties as assigned.
