Ohio Living Home Health & Hospice - Greater Cleveland is seeking a Physical Therapist to perform evaluations and treatments in patients' homes, following physician orders and state/federal guidelines.
You will educate patients and families, develop and adjust plans of care, participate in interdisciplinary teams, and maintain accurate records using EMR. Must have Ohio PT license and valid driver's license; new grads welcome, some home health experience preferred.
