Position: Registered Nurse (RN) Care Navigation Team Leader

Reserve Care, formerly Hospice of the Western Reserve, is a provider of hospice, palliative, and grief support services, honoring nearly 50 years of trusted service and committed to delivering compassionate care.

What You Will Do
  • Supervise Care Navigation staff including Registered Nurses (RNs), Licensed Practical Nurses (LPNs), Care Navigation Social Workers, and Care Navigation Support Agents.
  • Carry out additional supervisory assignments as directed by the Director of Navigation Services.
  • Participate in the planning and implementation of the Care Navigation Team Development Project.
  • Assist with onboarding and training of new Care Navigation staff.
  • Provide supervisory support for evening triage as needed.
  • Participate in quality, performance, and process improvement initiatives.
  • Cross‑train to provide coverage for additional Care Solutions Center leadership roles including On‑Call Teams, Admissions/Liaison, and Triage.
Success Capabilities
  • Registered Nurse (RN) in the state of Ohio strongly preferred.
  • Minimum of two years’ hospice experience required.
  • Strong communication, leadership, and managerial skills.
  • Ability to lead a large, multidisciplinary team, including managing performance and disciplinary action when necessary.
  • Strong technology proficiency.
  • Proven ability to manage multiple priorities and operational demands.
Physical Requirements

This is sedentary work requiring the exertion of up to ten (10) pounds of force occasionally and some light work and lifting requiring the exertion of up to 20 pounds occasionally. Work requires sitting, stooping, bending, kneeling, reaching, standing, fingering, grasping and repetitive motions; vocal communication; hearing, and visual acuity. Employee is not subject to adverse environmental conditions. This position has the potential to be exposed to environmental elements, internal and external, such as infectious disease, blood borne pathogens, and chemical and/or chemical fumes, pet dander, and exposure to allergens, and adverse effects of inclement weather that may occur as well as potentially difficult patient and family situations.
