SUMMARY

The Licensed Practical Nurse (LPN) provides comprehensive skilled care as ordered by the attending physician. The LPN follows order sets created by the RNCM within a specified geographical territory, educates patients and families, ensures patient safety, documents accurately, and remains compliant with all legal rules and regulations. The LPN performs all duties within the scope of practice outlined by the state.

QUALIFICATIONS

& ATTRIBUTES
  • Minimum of one year of experience as an LPN in an acute‑care setting (two years preferred). Home‑care experience desired.
  • Effective communication skills and the ability to create positive impressions with patients, families, physicians, and co‑workers.
  • Calm, patient, accommodating, and compassionate when working with patients.
  • Knowledgeable about nursing best practices and capable of making appropriate nursing judgments.
  • Ability to identify situations and respond with the best possible solution.
  • Detail‑oriented, observant, and disciplined with a strong work ethic.
  • Ability to prioritize tasks, meet deadlines, and follow directions as a team member.
  • Valid driver’s license and auto insurance with the employee’s name listed as the driver.
MAJOR AREAS OF RESPONSIBILITY
  • Provides skilled nursing care in a patient’s home as ordered by the attending physician.
  • Performs care by following order sets and goals set by the RN.
  • Observes and monitors patient condition, notifying the RNCM of any changes.
  • Administers medication as prescribed by the physician.
  • Counsels patients and families on disease/injury processes and pain management.
  • Incorporates patients and families into development of the plan of care.
  • Helps decrease rehospitalizations by teaching preventative measures and decision‑making skills.
  • Listens to patients and family members.
  • Completes all clinical documentation per agency protocol and Medicare/Federal guidelines.
  • Maintains an active LPN license in the state of employment.
  • Communicates with scheduler regarding any changes outside the normal schedule.
  • Participates in all on‑call requirements and case conferences.
  • Attends in‑service training and mandatory agency meetings.
  • Makes a professional appearance as a representative of the company.
  • Understands and practices agency policies and procedures.
ESSENTIAL FUNCTIONS
  • Drives to patients’ primary locations per scheduled visit.
  • Attends daily visits at assigned locations.
  • Documents all aspects of visits within 24 hours of each visit to meet Medicare guidelines.
  • Submits all required documentation within specified company requirements.
  • Provides skilled care within the scope of practice for an LPN.
  • Fulfills LPN visits in assigned geographic locations per patient need within federal and state specifications.
  • Per visit compensation is a lump‑sum payment covering patient care, preparation, travel, labs, supplies, charting, and communication with patients, families, physicians, and other healthcare providers.
HEALTH QUALIFICATIONS

Health Requirements:
Must meet all physical position requirements. Evidence of a current negative tuberculin skin test or X‑ray (or completion of TB testing upon orientation) is required; thereafter an annual tuberculin skin test is required unless previous documentation exists. Employees must be free of physical or medical conditions limiting their ability to perform job functions. This position may expose employees to blood borne pathogens.
