• Licensed Practical Nurse with current license in the state of employment
• Minimum 1 year of experience as an LPN in an acute care setting (2 years preferred)
• Home care experience desired
• Effective communication and positive impression with patients, families, physicians, and co‑workers
• Calm, patient, and accommodating under stress
• Compassionate and caring with patients
• Knowledgeable in nursing best practices
• Ability to make appropriate nursing judgments
• Detail‑oriented and observant
• Valid driver’s license and auto insurance with your name as a listed driver

• Provides skilled nursing care in a patient’s home as ordered by the attending physician
• Follows order sets and goals made by RN
• Observes and monitors patient conditions
• Notifies RNCM of any changes in patient condition
• Administers medication as prescribed by the physician
• Counsels patient and family on disease/injury processes and management
• Incorporates patient and family in development of plan of care
• Teaches patients preventive measures to reduce re‑hospitalizations
• Listens to patient and family members
• Completes all clinical documentation in accordance with agency protocol and Medicare/Federal guidelines, documenting all aspects of treatment, assessments, and patient care

• Drives to patient’s primary location per scheduled visit
• Daily attendance at assigned visit locations
• Documents all aspects of subsequent visits within 24 hours to meet Medicare guidelines
• Completes and submits all required documentation within specified company requirements
• Knowledgeable with providing skilled care within the scope of practice for a Licensed Practical Nurse
• Fulfills LPN visits in assigned geographic location per patient need within federal and state specifications
