Field Registered Nurse; RN - Optum Senior Community Care - Cuyahoga County; Western

As a team member of our Senior Community Care (SCC) product, we work with a team to provide care to patients at home in a nursing home, assisted living for senior housing. This life‑changing work adds a layer of support to improve access to care. We’re connecting care to create a seamless health journey for patients across care settings.

You will be part of the primary care team collaborating with all interdisciplinary team members and will support multiple providers. The RN is a flexible team member who works alongside nurse practitioners (NPs) and physician assistants (PAs).

Hours:

Monday‑Friday 8:00 a.m. to 5:00 p.m. EST

This is a field‑based position traveling to facilities throughout Cuyahoga County (i.e., primarily Western Cleveland, Parma, & North Royalton).

• Responsible for collaborating with all members of the interdisciplinary care team around changes in condition and/or care coordination
• Triage member needs to the appropriate care team member and level of care
• Assist with transitions in care: outpatient and inpatient
• Assist in assuring complete and accurate documentation, medication reconciliation, nursing rounds, and risk assessments

• Active unrestricted Registered Nurse license in your state of residence or ability to obtain within 60 days of hire
• Certified in Basic Life Support
• Driver's license and access to reliable transportation that will enable you to travel up to 100% to visit clients and/or patient sites within a designated area

• BSN degree
• 1+ years of experience working as an RN (long‑term care setting preferred)
• Experience in geriatric medicine, long‑term care, senior living or home care setting

The salary for this role will range from $60,200 to $107,400 annually based on full‑time employment. We comply with all minimum wage laws as applicable.
