Job Qualifications

Education: Graduate of an accredited school of nursing.

Licensure: Valid license as a Registered Nurse. Current Basic Life Support certification. Valid state Driver’s License.

Experience: Hospice or Home Health experience preferred. Minimum one year of recent professional nursing experience.

  • Operate within policies, procedures, state and federal regulations, and patients’ plans of care while managing and caring for Traditions Health patients after business hours.
  • Cope with emotional stress and tolerate individual lifestyles.
  • Be sensitive to the needs of patients with advanced illness and families, and own feelings about dying and death.
  • Work independently.
  • Good interpersonal skills.
  • Computer skills including Microsoft Word and e‑mail.
Transportation

Reliable transportation and valid/current driver’s license and auto insurance. Staff required to work in patients' homes and be responsible for own transportation.

Environmental and Working Conditions

Works in customers’ offices in various conditions. Ability to travel to customers’ locations, some overnight stays, and exposure to unpleasant weather. Staff may be exposed to viruses, disease and infection from patients and specimens.

Physical and Mental Effort

Prolonged sitting and some standing. Occasional need to lift, pull, carry, and push items up to 50 lbs. Stressful conditions to meet deadlines. Excellent problem‑solving skills.

Essential Functions
  • Be available by phone or pager to meet patient needs during assigned shift.
  • Identify and coordinate patient/family care to support patients with advanced illness in home, skilled nursing facility or residential care facility.
  • Document and communicate interventions and plan changes to the Case Manager.
  • Provide comfort care and maximize quality of life for patients and families through teaching, assessment, and intervention.
  • Assess hospice needs, consider physical, emotional, spiritual aspects, and gather data on social, economic, cultural factors influencing health and well‑being.
  • Receive reports from nursing staff.
  • Respond promptly to answering service and voicemail calls.
  • Triage patient calls and respond appropriately via phone or visit.
  • Implement plan changes with interdisciplinary collaboration, medical director or attending physician.
  • Document all calls, interventions, care plan changes, and physician contacts per policy.
  • Update patient status and plan of care in electronic, verbal, and written formats.
  • Document nursing interventions, patient responses, and teaching evidence.
  • Assist families and caregivers with empathy, respect confidentiality, and communicate courteously.
  • Coordinate services with vendors, outside agencies, and interdisciplinary team.
  • Report orders or service requests and follow through.
  • Attend patient deaths to support families, contact mortuary, advise on narcotic disposal, notify IDT and vendors.
  • Provide bereavement resources to families as appropriate.
  • Respond to referral calls, provide hospice information, screen for appropriateness, collect patient info, and coordinate with admin on‑call.
  • Perform patient intakes and information visits as needed, including explanation of hospice benefits/Medicare, full assessment, and paperwork.
  • Perform patient admission visits as assigned. Report concerns to on‑call supervisor.
  • Perform duties outside regular hours per on‑call rotation.
  • Participate as a member of Traditions Health team and its mission and core values.
  • Provide point‑of‑service electronic charting and documentation on visit or admission. Proficiency with Windows‑based software required.
  • Submit documentation within company policy.
