Nurse Practitioner / Physician Assistant (NP/PA) — Greater Cleveland Area

About UsElaya Health is a physician practice serving skilled nursing facilities (SNFs). We're looking for a dedicated NP or PA licensed in Ohio to join our team and provide care to patients in the Greater Cleveland Area.

- Conduct patient assessments, manage chronic conditions, and follow up on treatment plans
- Collaborate with facility staff and supervising physicians to coordinate patient care
- Document visits and maintain accurate patient records
- Active NP or PA license in the State of Ohio
- At least 1 year of endocrinology experience (required)
- Experience in a SNF, long-term care, or geriatric setting a plus
- Strong communication skills and ability to work independently
