About The Role

What We Are Looking ForRequired Qualifications
- Current, unrestricted Registered Nurse (RN) licensure in the state of practice
- Basic Life Support (BLS) certification
- Demonstrated experience in psychiatric and mental health nursing, including medication management, counseling support, and clinical assessment
- Proficiency with Electronic Medical Records (EMR);
Epic experience preferred - Ability to administer intramuscular (IM) injections, including long‑acting injectable (LAI) treatments
- Certification in Psychiatric‑Mental Health Nursing (PMH‑BC) or as a Psychiatric‑Mental Health Nurse Practitioner (PMHNP‑BC)
- Experience with clozapine management and STARS/REMS systems
- Familiarity with OARRS (Ohio Automated Rx Reporting System) or equivalent state prescription drug monitoring programs
- Prior experience in community mental health or outpatient psychiatric settings
You will conduct thorough assessments of patients' physical status, mental status, and psychiatric symptoms — including evaluation of psychotic symptoms and safety factors. You will screen and monitor patients for medical problems and medication side effects, and provide comprehensive Registered Nurse Care appointments, including long‑acting injection (LAI) treatments.
Medication ManagementYou will assess medication adherence, evaluate refill needs, and manage the full medication system, including administration, documentation, and proper storage and disposal according to organizational policy. You will also coordinate with the program psychiatrist to initiate and stabilize patients on medications, access the OARRS report for patients on controlled medications, and obtain urine drug screens as ordered by the provider.
Coordination & CollaborationYou will collaborate with shared provider appointments — particularly for patients experiencing instability, needing medication changes, or presenting with side effects. You will provide Bridge Appointments following hospital discharge, including review of discharge orders, shared provider appointments, entry of new orders and hospital information into Epic, and patient scheduling coordination with their primary care physician (PCP). You will also communicate and coordinate services with other medical providers, including clozapine resources and STARS consultation.
Education& Health Promotion
You will meet individually with patients to provide comprehensive medication education, monitor effectiveness, and develop systems to support medication compliance — involving family and significant others as appropriate. You will engage in health promotion, prevention, and education activities, including risk behavior assessment and behavior change support. You will also educate team members on monitoring psychiatric symptoms and medication side effects.
Clinical & Administrative ResponsibilitiesYou will utilize EMR systems including Epic, REMS, OARRS, and Clini Sync. You will provide triage line coverage, participate in clinical team meetings and treatment planning, and support the development of new services within the program.
