FT Hospice RN Case Manager

Job Summary

National Church Residences Home Health division is hiring a Full‑Time RN Case Manager. The position requires Monday to Friday 8:30 a.m. to 5:00 p.m., with occasional on‑call weekends and holidays. Mileage reimbursement is $0.72 per mile.

The RN Case Manager will primarily handle admissions and coordinated care planning, working closely with interdisciplinary teams including physicians, LPN/LVNs, and aides to develop and implement individualized care plans for seniors.

• Develop, implement, and update individualized care plans, coordinating with all involved physicians and integrating orders.
• Include client, caregiver, and representative in planning, provide education, mentoring, and support.
• Use health assessment data to determine problems, goals, and interventions.
• Communicate with community health providers and facility staff to coordinate care plans.
• Establish, monitor, and document ongoing home health eligibility.
• Perform initial assessments and complete full physical assessments with medical history.
• Initiate preventive, rehabilitative, and nursing procedures; may administer medications and treatments as prescribed.
• Prepare clinical notes, update physicians, and provide direct client care per state practice act.
• Educate and mentor clients and caregivers regarding care needs.
• Provide support and education on end‑of‑life issues and care.
• Re‑evaluate client nursing needs regularly; adjust plans as status changes.
• Coordinate discharge planning with interdisciplinary members.
• Participate in scheduled interdisciplinary team meetings and ensure proper documentation and communication.
• Supervise LPN/LVNs, household/hospice aides as per policy and competency evaluations.
