• Pay: $63,250 - $96,467.50 annually

• Schedule: Full-time, Day shift

• Work collaboratively with a multidisciplinary care team for high-risk patients.
• Outline the nature and duration of involvement needed by the specialty care team.
• Utilize technological tools to manage patient populations.
• Identify patients with actual or potential care needs requiring coordination.
• Conduct targeted outreach to high-risk patients for timely care delivery.
• Provide care and disease management coordination.
• Conduct comprehensive clinical assessments of patients' needs.
• Inform and work with patients and families regarding care coordination.
• Serve as a liaison and advocate for patients and families.
• Assist in managing transitions of care across settings.

Education:

• Graduate from an accredited school of Professional Nursing
• Bachelor's of Science in Nursing (BSN) preferred

Experience:

• 3-5 years of nursing experience

Licenses &

Certifications:

• Current state licensure as a Registered Nurse (RN)
• Basic Life Support (BLS) Certification required
