RN Hospice Coordinator - Main Campus and Eastside Region

About This Role

The RN Hospice Coordinator at Cleveland Clinic's Main Campus is responsible for ensuring a smooth transition for patients from hospital to home or facility, providing a full continuum of care. This role is ideal for compassionate nurses who excel in coordinating services and enhancing patient quality of life.

• Pay: $30.41 - $46.38/hour
• Location: Cleveland, OH
• Facility: Cleveland Clinic Main Campus
• Department/Unit: Main Campus
• Schedule: Full-time, Day shift

• Act as a member of the interdisciplinary team for coordination of services.
• Process inpatient referrals into hospice and ensure continuity of care.
• Actively educate potential customers about hospice care benefits.
• Ensure patients and families understand the hospice benefit.
• Process inquiries for all new referrals (internal and external).
• Ensure seamless transition from hospital to community by ordering medical supplies and DME.
• Perform assessments of potential patients consistent with hospice policies.
• Plan delivery of patient care in collaboration with other disciplines.
• Assess patient for safety risks, abuse, and neglect.
• Document changes in patient condition and effectiveness of care/interventions.

Education:

• Graduate from an accredited school of Professional Nursing
• Bachelor’s of Science in Nursing (BSN) preferred

Experience:

• 1+ years of current clinic experience as an RN
• 1+ year of experience in an acute med/surg or critical care setting

Licenses &

Certifications:

• Current state licensure as a Registered Nurse (RN)
• Basic Life Support (BLS) Certification through AHA or American Red Cross
