Registered Nurse; RN; Home Health

We are seeking a skilled and compassionate Registered Nurse (RN) for a home health role dedicated to delivering high-quality, patient-centered care.

• Conducting in-home patient visits, including assessments, health evaluations, and clinical interventions such as medication administration, wound care, and testing.
• Supporting care transitions, end-of-life discussions, and addressing quality gaps through screenings and education.
• Collaborating with interdisciplinary teams, including physicians and nurse practitioners, to develop and implement personalized care plans.
• Maintaining accurate documentation, ensuring compliance with regulations, and supporting telehealth consultations.
• Building positive relationships with patients, caregivers, and community providers.

• Active state RN licensure; prior home care, hospice, palliative, or geriatrics experience preferred.
• Minimum of 3 years' direct patient care experience; CPR certification required.
• Proficiency with electronic health records, telehealth platforms, and Microsoft Office Suite.
• Strong communication, organizational, and decision-making skills.
• Ability to work independently, travel locally, and lift up to 50lbs.

This role involves local travel, administrative duties, and participation in team meetings to ensure comprehensive patient care in a supportive, patient-first environment.
