Nurse Practitioner – In-Home Health Evaluations

We are seeking 1099 nurse practitioners (NPs) to perform in-home health assessments for Medicare Advantage, Medicaid, and commercial plans.

Key responsibilities include:

• Conducting pre-scheduled visits using structured workflows and pre-populated forms
• Completing assessments without treatment plans, prescriptions, or follow-up calls
• Submitting forms efficiently to complete each visit
• Managing a high volume of patient assessments across multiple regions

Qualifications and skills required:

• Active, unrestricted state NP license
• Board certification in FNP, AGNP, or ANP
• Availability for at least 60 hours per month
• Open to new graduates without prior experience
