This RN role supports the complex hepatology patients, improve patient access and care coordination, support providers in order to increase efficiency, and ensure management of chronic liver disease both in and outside of scheduled clinic appointments.
-Triage and management of patient phone calls, portal messages, and clinical inquiries.
-Coordination of care plans, diagnostic testing, imaging, and laboratory monitoring.
-Education and counseling for patients with liver disease, cirrhosis, hepatitis, fatty liver disease, and transplant-related needs.
-Medication management, prior authorizations, and monitoring of treatment adherence and outcomes.
-Coordination with primary care providers, specialists, infusion services, transplant centers, and ancillary departments.
-Review and follow-up of laboratory results and diagnostic studies.
-Ongoing patient management outside of clinic visits to ensure timely interventions, continuity of care, and reduction of avoidable hospitalization

• (BSN) Bachelor's Degree in Nursing (Required) or
• (MN) Master in Nursing (Required)

• Knowledge of nursing process and practice. (Required proficiency)
• Uses a wide variety of technical equipment to meet patient needs and prescribed clinical therapies (ie. stethoscopes, thermometers, sphygmomanometers and electronic monitoring devices. (Required proficiency)

• Registered Nurse (RN), Ohio and/or Multi State Compact License (Required Upon Hire) and
• Basic Life Support (BLS) (Required Upon Hire)
• Advanced Cardiac Life Support (ACLS) May be required based on the clinical care standards of the unit. Refer to ACLS/PALS Unit Requirements document found on DWP Nursing Portal to determine requirements by unit.
• Pediatric Advanced Life Support (PALS) May be required based on the clinical care standards of the unit. Refer to ACLS/PALS Unit Requirements document found on DWP Nursing Portal to determine requirements by unit.
