We are seeking a dedicated and compassionate Evening Shift Licensed Practical Nurse (LPN) to join our team!

As an Evening Shift Licensed Practical Nurse (LPN) at Harvard Gardens Rehabilitation & Care Center, you will be responsible for providing high-quality patient care to our residents and patients. You will work closely with our interdisciplinary team to develop and implement individualized care plans, administer medication and treatments, and maintain accurate records. This is a full-time position with flexible scheduling to fit your needs.

• Deliver high-quality patient care to our residents and patients, focusing on their physical, emotional, and spiritual needs
• Develop and implement individualized care plans in collaboration with the interdisciplinary team
• Administer medication and treatments as prescribed by physicians
• Maintain accurate and detailed records of patient care and treatment
• Collaborate with other healthcare professionals to ensure seamless care transitions
• Provide education and support to patients and their families
• Participate in quality improvement initiatives to improve patient outcomes and satisfaction

• Current LPN licensure in the state of Ohio
• Minimum one year of experience in a long-term care or rehabilitation setting
• Strong clinical and communication skills
• Ability to work effectively in a fast-paced environment
• Compassionate and patient-focused attitude
