MDS RN​/LPN

St. Augustine Manor, a 220-bed skilled nursing facility and part of St. Augustine Health Ministries, is looking for a detail-oriented MDS nurse to join our clinical leadership team. This is a Full-Time, Monday-Friday, day shift position.

• Complete accurate, timely MDS 3.0 assessments (admission, quarterly, annual, significant change, discharge, and PPS) in line with the CMS RAI Manual.
• Coordinate with nursing, therapy, dietary, social services, and activities to make sure assessments reflect each resident’s actual condition.
• Oversee the care planning process and ensure care plans are individualized, current, and consistent with MDS findings.
• Track assessment schedules, submit on time, and resolve validation errors.
• Monitor PDPM coding, Quality Measures, and Five-Star indicators, and flag documentation gaps before they become survey or reimbursement problems.
• Support survey readiness

• Current Ohio RN or LPN license
• At least [1–2] years of MDS experience in long-term care or post-acute.
• RAC-CT certification preferred (or willingness to obtain it).
• Strong working knowledge of PDPM, Section GG, and Section M.
• Experience with PCC a plus.
• Excellent organization and attention to detail
