RN Care Coordinator - Pediatric Cardiology

• Schedule: Full-time, Day shift

• Work collaboratively with a multidisciplinary care team to develop goals and plan interventions.
• Provide care and disease management coordination.
• Identify patients with ongoing coordination needs and conduct targeted outreach.
• Conduct comprehensive clinical assessments addressing medical, behavioral, pharmacy, social, and end-of-life needs.
• Inform and work with patients and families regarding care coordination, education, and coaching.
• Monitor patient compliance with care plans and perform reassessments of patient progress.
• Serve as a liaison and advocate for patients and families.
• Assist in managing transitions of care across settings, ensuring optimal communication.
• Identify barriers, facilitate solutions, and connect patients to community resources.

• Graduate from an accredited school of Professional Nursing
• Bachelor’s of Science in Nursing (BSN) preferred

• 3-5 years of nursing experience

• Current state licensure as a Registered Nurse (RN)
• Basic Life Support (BLS) Certification required
