Garden Springs Healthcare is seeking an experienced, relationship-driven Hospital Liaison to support our continued growth throughout the Greater Cleveland market
.
This is a market-focused business development role responsible for representing the Garden Springs Healthcare network and strengthening our partnerships with hospitals, health systems, case management teams, social workers, physicians, and other referral partners throughout the region.
The ideal candidate understands the Cleveland healthcare landscape, has established relationships within local hospitals, and knows how to turn those relationships into meaningful referral and census growth.
Position OverviewThe Hospital Liaison will serve as a primary representative of Garden Springs Healthcare throughout the Greater Cleveland hospital market. Rather than representing a single facility, this position will promote the capabilities of all Garden Springs facilities serving the market
, helping referral partners identify the most appropriate placement based on each patient's clinical, geographic, and insurance needs.
This individual will work closely with our facility leadership, clinical teams, Centralized Admissions & Intake, and corporate business development team to create a seamless referral and admission experience.
Key ResponsibilitiesDevelop and maintain strong relationships with hospital case managers, social workers, discharge planners, physicians, utilization review teams, and other key referral sources throughout Greater Cleveland.
Represent the full Garden Springs Healthcare portfolio and educate referral partners on our clinical capabilities, specialty programs, geographic coverage, and payer options.
Identify new hospital relationships and opportunities to expand Garden Springs' presence within existing accounts.
Maintain a consistent presence within assigned hospitals and become a trusted resource for complex discharge planning and post-acute placement.
Drive referral volume, admission conversion, and overall census growth across the Garden Springs network.
Serve as a resource for challenging or complex placements and help referral partners identify appropriate solutions within the Garden Springs system.
Collaborate closely with Centralized Admissions & Intake to ensure referrals receive timely review, communication, and follow-up.
Work with facility teams to coordinate admissions and maintain strong communication with hospital partners throughout the transition of care.
Conduct appropriate bedside visits and patient/family meetings when needed to support placement and successful admissions.
Track referral patterns, lost opportunities, competitive activity, and opportunities for additional growth within assigned accounts.
Participate in hospital meetings, networking opportunities, community events, and other business development activities.
Maintain strong follow-up after admission to strengthen relationships and support future referrals.
Partner with leadership to develop and execute market-specific strategies for continued census and specialty-program growth.
Previous experience in skilled nursing, post-acute care, hospital business development, healthcare sales, admissions, case management, or a related healthcare role.
Strong knowledge of the Greater Cleveland healthcare market and established hospital relationships strongly preferred.
Proven ability to develop referral relationships and drive measurable growth.
Understanding of the post-acute referral and discharge-planning process.
Ability to confidently communicate clinical capabilities and placement solutions to healthcare professionals.
