Your Contribution To Our SuccessA Housing Case Manager contributes to AHF by leveraging strong experience in Social Services and understanding the social determinants affecting people living with HIV/AIDS, as well as knowledge of Social Service Programs within Cuyahoga County. You will collaborate with all teams in the Cleveland office so that every patient receives the support they require and access to available resources. You will assist patients with housing and utility needs, ranging from helping with rent arrears to avoid eviction to facilitating permanent housing and/or utility assistance, all while keeping the patient in care.
You will be a passionate advocate for our top initiatives and ensure that the AHF core Values and Mission stay top of mind in a dynamic environment. The Housing Case Manager will work as part of a close-knit team to meet clients where they are, identify strengths and opportunities for growth, and assist them with realistic goal setting.
- Strong experience in Social Services and an understanding of the social determinants affecting people living with HIV/AIDS.
- Thorough understanding of Social Service Programs within Cuyahoga County.
- Ability to work with all teams in the Cleveland office to coordinate patient support.
- Passionate advocacy for organization initiatives and alignment with core values and mission.
- Ability to adapt in a dynamic environment to provide the best experience for clients and employees.
