Case Coordinator

The Case Coordinator provides direct support to veterans residing in a residential facility by assessing needs, coordinating services, and promoting stability and successful outcomes. Working collaboratively with veterans, interdisciplinary team members, and community partners, the Case Coordinator develops individualized service plans that are driven by each veteran's identified goals and needs.

The Case Coordinator assists veterans in accessing housing, employment, and income support services while providing ongoing care coordination to promote long-term stability. This role is responsible for connecting veterans to appropriate resources, monitoring progress, and ensuring accurate and timely documentation. Strong communication, organization, and problem-solving skills are essential to effectively support veterans with diverse and complex needs.

Key Responsibilities:

  • Provide direct support and case management services to veterans residing in a residential facility.
  • Assess veteran needs and develop individualized service and goal plans based on identified needs, strengths, and goals.
  • Assist veterans with obtaining and maintaining housing, employment, and income support services.
  • Connect veterans with community resources, benefits, and supportive services to promote stability.
  • Provide ongoing care coordination and monitor progress toward established goals.
  • Maintain accurate and timely documentation, including case notes, service plans, goal plans, and other required records.
  • Collaborate with internal team members and external partners to coordinate services and address veterans' needs.
  • Transport veterans to appointments, services, and other approved activities, as needed.
  • Maintain regular communication with veterans to assess progress, identify barriers, and adjust service plans accordingly.
  • Participate in team meetings, trainings, and other program activities.

Applicants must have at minimum an associates degree and experience in social services or Bachelors degree. Valid drivers license is required.
