Position Summary
The Family Overflow Monitor is responsible for overseeing services for clients and families utilizing overflow family shelters operated by LMM. This position is part of the Housing & Shelter Family Overflow and Housing teams. This position engages families experiencing homelessness to assess needs and link to admit shelter at partner sites or direct placement into housing. The goal of the position is to ensure homelessness is brief, rare, and a one-time experience and avoid homelessness whenever possible through directly providing high quality and responsive case management for families.
Some of the duties include:
- Work collaboratively with Family Overflow Supervisor and team of staff provide shelter operations and security whether congregate site based or scattered site based.
- Support clients in creating Exit Plans with Shelter staff to ensure rapid exit from homelessness and linkage to appropriate destinations
- Link and refer clients with resources and to community partners including obtaining identification, benefits and entitlements, mental health and substance use disorder services and housing interventions
- Works closely with emergency shelters to assure placements and transfers are made efficiently and appropriately
- Works closely with Coordinated Entry regarding moves from overflow to admit shelters
- Assist in arranging transportation to critical destinations (e.g. housing, medical, employment)
- Maintain participant data and statistical information, client database records, Housing Assessments, and progress notes in HOPE
- Maintain regular contact with appropriate partner agency staff for individual client/staffing updates
- Collaborate with Housing & Shelter Clinical team regarding mental health and substance use crises
- Assist to facilitate any Shelter emergencies that occur, according to program policy, ensuring the safety of all shelter clients and staff
- Conduct living area inspections on a regular basis when meeting with families at shelter/hotel sites
- Assist in distribution of clothing and hygiene items as available to program participants
Qualifications
Our ideal candidate has a high school diploma or equivalent; some experience working with the homeless, chemically dependent, or mentally disabled individuals; and effective communication and crisis intervention skills.
