Position: LSW Case Manager
Location:
Cleveland, Ohio
Base salary: $61,547.20 per year. New hire bonus: $5,000. Total compensation: $66,547.20.
Position SummaryAs an LSW Case Manager, you will support long‑term clients age 60 and older enrolled in the MyCare Ohio Care Source waiver programs through comprehensive assessment, care coordination, advocacy, and ongoing case management. This opportunity combines community impact, relationship‑driven service, and hybrid field‑based flexibility while helping individuals remain in the place they call home.
Responsibilities- Conduct comprehensive client assessments
- Develop and implement individualized care plans

- Current licensure as a Licensed Social Worker (LSW) through the State of Ohio Counselor and Social Work Board
- Minimum of one (1) year of experience working with individuals with physical or behavioral disabilities in home health care, medical social work, or behavioral health environments
- Excellent communication, interviewing, and assessment skills
- Knowledge of community resources available to older adults and individuals with disabilities
