The organization helps parents break cycles of generational poverty, early parenting, and incarceration by providing parenthood enrichment, workforce development training, and comprehensive case management. Guided by a vision where every child has nurturing, engaged parents, Passages emphasizes the profound impact of raising children who understand their ability to contribute positively to the world. Initially focused on low‑income fathers, including non‑custodial and reentry populations, Passages now serves all low‑income parents facing socio‑economic and family challenges in Northeast Ohio.

Role Description

The Case Manager/Team Lead is a full‑time, on‑site role located in Painesville, Ohio. This position provides direct case management services to parents, including intake assessments, individualized service planning, referrals to community resources, and ongoing progress monitoring. The Case Manager/Team Lead coordinates parenthood enrichment and workforce development activities, supports participants in overcoming barriers to employment and family stability, and maintains accurate documentation in accordance with program standards.

The role includes leading and mentoring a small team of staff or volunteers, facilitating team meetings, and ensuring consistent, high‑quality service delivery. The Case Manager/Team Lead collaborates with community partners, participates in outreach, and contributes to program evaluation and improvement efforts.

Qualifications
  • Strong case management and counseling skills, including assessment, goal‑setting, and service coordination.
  • Experience in family support, parenting education, or social services, especially with low‑income or justice‑involved populations.
  • Team leadership and supervision abilities, including mentoring staff, organizing workflows, and supporting professional development.
  • Excellent communication, documentation, and interpersonal skills, with the ability to build trust and rapport with diverse participants.
  • Knowledge of local community resources, workforce development programs, and systems affecting families (e.g., child support, courts, social services).
  • Ability to manage time effectively, handle multiple cases, and maintain confidentiality and ethical practice standards.
  • Bachelor’s degree in social work, human services, psychology, or a related field preferred; equivalent experience will be considered.
  • Experience working in urban communities and a demonstrated commitment to equity, inclusion, and non‑discriminatory practices.
  • Proficiency with basic computer applications and case management or data tracking systems.
  • Availability to work on‑site in both Lake and Ashtabula Counties, and occasionally adjust hours for participant or community needs.
