Our Permanent Supportive Housing (PSH) program combines low-barrier, affordable housing with integrated health care and wraparound supportive services — helping individuals and families who have experienced chronic homelessness build the stability they deserve.
We are looking for a compassionate, skilled, and mission-driven Case Manager to join our PSH Care Team. If you are someone who believes in the dignity and resilience of every person you serve, and who brings evidence-based practice to life through genuine human connection, we want to hear from you.
What You Will DoAs a Case Manager in our Permanent Supportive Housing program, you will provide strength-based case management services to individuals with histories of chronic homelessness, behavioral health conditions, and complex psychosocial needs. Your work will be rooted in whole-person care — addressing not just immediate needs, but the medical, behavioral health, substance use, and social determinants of health factors that shape long-term wellness and housing retention.
Engagement and Client Support You will build and sustain meaningful, therapeutic relationships with clients through face-to-face, telephonic, office-based, property-based, and community-based engagement. You will explain the purpose and process of case management to clients, conduct comprehensive assessments of strengths and needs, administer appropriate screening tools, and support clients in developing the independent living skills — including budgeting, understanding leases, and paying bills — that promote lasting housing stability.
Service Planning and Case Management You will develop and maintain individualized service plans and housing success plans that reflect client goals, measurable outcomes, and evidence-based strategies. You will apply Housing First, Harm Reduction, Motivational Interviewing, Stages of Change, and Stage-Wise Interventions to support wellness and housing retention. You will also provide ongoing life skills training and pro-social support, individually and in groups, and conduct continuous monitoring to evaluate progress and adjust interventions.
Coordination, Linkage, and Monitoring You will work side-by-side with a multidisciplinary team — including counselors, nurses, physicians, pharmacists, social service staff, and Property Management — to connect clients to the full range of community resources they need: primary care, behavioral health, substance use services, benefit programs, educational supports, vocational resources, and social networks. You will assist clients with benefit applications, advocacy, and appeals, and support smooth transitions in care following hospitalizations or other disruptions.
Clinical Proficiency and Documentation You will recognize behavioral health, physical health, and substance misuse presentations, apply appropriate risk and lethality assessment practices, and use clinical supervision effectively. You will document all case management activity in the organization's electronic medical record system in a clear, objective, and timely manner, ensuring that documentation directly links services to client goals and outcomes. You will protect client privacy and confidentiality in strict accordance with applicable laws and organizational policy.
Site and Program Support You will contribute to the vitality of the PSH community by participating in multidisciplinary team meetings, housing inspections, move-in and move-out planning, and community-building activities. You will also support onsite food pantry operations approximately once per month and maintain required food safety certification as provided through The Centers.
