Front Line Service in Cleveland, OH is seeking a Community Transition Program (CTP) Case Manager to provide supportive services and case management to adults transitioning from an institution to homelessness, with a substance use diagnosis. You will complete progress notes and maintain documentation per agency standards.
This is a non-exempt, union position with a starting wage of $21.00 per hour. The hours are 1st shift, Monday through Friday.
