Position Summary
The LPN promotes resident care, encourages a team approach with resident assistants, certified nursing assistants, and other team members in activities, housekeeping and dining services. The LPN provides direct care and directs and oversees care provided by caregivers to ensure the resident’s plan of care is followed. An active approach to resident care is taken, not just passive maintenance. LPNs are deeply involved in resident activities and programming and must understand and have a desire to grow in dementia‑specific care.
Essential Duties and Responsibilities- Support the maintenance of wellness objectives, standards of wellness practice, and wellness policies and procedure manuals.
- Identify ongoing needs and services of residents through the assessment/service plan process including documentation for residents with a change in health care status.
- Respond promptly to resident needs.
- Provide or assist residents with activities of daily living as needed, including bathing, oral care, toileting, dressing, and perform restorative nursing techniques such as proper positioning, ADL training, and passive range of motion.
- Assist with appropriate information and behavioral prompts, cues, redirection, and affirmative support to residents in need.
- Support hydration programs and good dining experience and dietary intake.
- Maintain a restraint‑free environment at all times and support fall prevention programs for all residents in need.
- Assist in post‑mortem care as instructed.
- Communicate appropriate and timely information to families as indicated and report family concerns to supervisor or director of nursing.
- Oversee and manage medication delivery system including timely order entry, receipt, tracking, daily MAR documentation, narcotic count, and checking in medications to ensure resident compliance.
- Maintain infection control policy and procedures, update and maintain manuals when indicated, and keep sufficient supplies on hand.
- Willingness to work beyond normal hours and in other positions temporarily when necessary.
- Coordinate with the food service director to ensure all residents receive diets prescribed by the physician.
- Maintain documentation reflecting wellness care provided, individual’s response to care, and disposition of each case.
- Arrange for competent, qualified team members to assume duties during any absence of the director of nursing.
- Participate in interviewing and selection of residents for admission and ongoing placement.
- Maintain open, effective communication with the community’s medical staff.
- Ensure a safe and sanitary environment for residents, team members, and visitors.
- Keep the director of nursing informed regarding work performance and any unusual incidents.
- Ensure confidentiality of all verbal and written information regarding residents, associates, and records.
- Demonstrate good judgment, strong problem‑solving and decision‑making skills.
- Attend in‑service and education programs and maintain professional certification or licensure.
- Perform other duties that may be assigned or needed.
- High School diploma or equivalent (GED).
- Current Licensed Practical Nurse license in the state of Tennessee.
- Competence in assessment skills, injection/medication administration, follow‑up and triage.
- Maintain current unrestricted state license as a Licensed Practical Nurse or a Professional Registered Nurse per state regulations.
- Excellent interpersonal, written and verbal communication skills.
- Knowledge of infection control policies and procedures.
- Ability to lift and move objects up to 50 pounds and transfer residents using proper body mechanics.
- Computer skills including Microsoft Windows, Word, Excel, email, and internet.
The employee is frequently required to use hands and fingers to touch, handle, and feel, to reach with hands and arms, and must be able to lift and move objects up to 50 pounds. Specific vision abilities required include close, distance and color vision, peripheral vision, depth perception, and ability to adjust focus. Must be able to lift and transfer residents using proper body mechanics.
Working environment may include occasional travel and exposure to therapy animals. Must work evenings and weekends maintain a flexible schedule, handle stress, and resolve conflicts.
